PLEASE NOTE: Proper packaging of returned items is your responsibility!!! THERE WILL BE ADDITIONAL CHARGES FOR ANY REPAIRS NEEDED DUE TO DAMAGE FROM IMPROPER PACKAGING.
- If you do not have a RMA # for this return, call 1-800-374-0933 to have a number issued.
- Complete the form below and include in your return package.
- Provide as much detail for return reason, especially if returning for warranty or repair service. Document any damage to the item(s). Any external damage to the product that is not documented above is your responsibility; therefore you may choose to insure your return package.
- Name, Address, and phone numbers are important in the event that we need to return an item to you or contact you regarding the return. If we will be shipping any items back to you, we will use the above name and address. If a different ship to address is needed, please indicate on a separate sheet of paper.
- Purchase Date: Items must be received at our 3360 PLEASANT HILL RD address within 30 days of the purchase date if you are taking advantage of our satisfaction guarantee refund. If returning warranty items please be sure you are returning an item within the a lot time of the product warranty. Please confirm that you are within the returnable time frame before returning an item.
- PACKAGING IS YOUR RESPONSIBILITY.
The RMA number must be plainly visible on the shipping label or the box.
